rough idle

rough idle
My 01 with 133k on it. At idle bout 800rpm it jumps prolly only bout 50rpms but enough to twitch the needle and the feeling is almost like an engine missing. what could my problem be.

My truck was doing the same thing when I was running the Power Service
Diesel Kleen.
