Needin' Some help with a 01' 24V

I hate to start off a post in a bad way - But I'd rather turn here to see what ya'll say about this issue I've got...I've been reading through the forums to see what I can dig up, and I'm still kind of lost as to what I should be looking at Replacing next.

My Truck is an 01 (was tuned with the V5 smartbox) Auto 4x4 QC,SB. Pushing 277k on the motor. Last week on my way to work, I pulled off the highway and TRIED (unsucessfully) to get back on the highway...I was dealing with the truck having a Dead pedal basically, Bucking and surging like it was being starved for fuel...Hard to believe with a full tank, so I went the code route. I DID change out the lift pump for a brand new one (the one that mounts to the side of the block just below the fuel filter housing) and that seemed to help it a little bit. I got the pump to prime up the first time I started it, and the pump doesnt seem to prime for longer than a couple seconds now...Not sure if that MIGHT be part of my problem or not though.

I got a code scanner from a buddy that I work with and it threw me these Codes:
P0113
P0118
P0123
P0343
P0606
P1286
P1475

Everything I've read leads me back to the P0606 code...Showing a fault internally with the ECU...But It CONSTANTLY throws the APPS code too..So I'm wondering what I should do next. I went through today and replaced all the ground cables on both batteries and checked the resistance with an Ohm meter, to which everything had little to no resistance so I am good there...And it's still doing it...The REALLY strange part is it only does this when it starts to heat up, or rather when the temp just BARELY starts to rise. I've cleared the codes, and let it run only to have it throw those same series of codes again.

I'm STUMPED! Anyone care to lend a hand?? PLEASE?

I would throw a "timbo apps" at it, reasonably priced and even if thats not the problem it probably will be soon. If you search Timbo apps on here you'll find where to get one. You also need a fuel pressure gauge to monitor.
