Speedo reading off

I'm experiencing the exact problem right now.
92 D350, 2WD, no change in tires.
Trans (auto) was just rebuilt by local shop.
When I drive by a radar, I have an speedo reading 40, but radar sign reads 48MPH. In the past, they've repeatedly been a dead match, so I trusted my SPEEDO and ODOMETER.
I suspect my odometer is also off since my full tank range is usually 350 (around town) miles, but last tank was 325 and it included a couple of long trips on freeway, so mileage should have been higher).
What is the COMMON part which can "slip" affecting both SPEEDO and ODOMETER?
Symptoms:
Bought truck with 58K reading on odometer, and note written on door frame to (add 350K to Odometer).. engine is actually strong..
Occasionally, SPEEDO would dance.
Recently SPEEDO was dancing and reading over speed but only momentarily
ODOMETER is reading low for usual tank full.
