I can comfortably tow grossing 16k up the steep passes in 3rd at 35-40 without what I feel is "wrapping" the engine out. I think I can get to about 45 pretty easy in 3rd. I wouldn't go to 4th under 40 unless you're just loafing.

What could be adjusted incorrectly to cause third not to put out much power up above 35mph ?
I haven't messed with the fuel pin, star wheel or smoke screw yet, just the idle and the fuel screw. There is probably a good write-up somewhere on here with pics on just this topic...
That, and I'll read up on the smoke screw, fuel pin and star wheel, too, maybe shoot for that as well. Sounds like I should top out in third at more than 35-40mph with the engine revving way high.
Before you go messing with re-indexing the throttle-lever, take pictures of it after each little piece is removed, and make good note of the lever's exact position in relation to the little indexing marks.
The splines on the throttle are very small and easy to lose track of where you started from.
If you are not 100% familiar with what is going on, I recommend you first study several of the threads we have on the VE-pump where there are plenty of pictures of the throttle indexing procedure.
Before messing with it, check the break-over spring and it's pins; more are worn in two and no longer functioning than not.
If that be the case, I would order and have on hand those parts before taking it apart.
AND, only "fix" one thing at a time, then test; otherwise, you won't have any way of knowing what helps and what hurts.

Today, I moved the throttle shaft one index mark counter-clockwise on the lever. I tried two and three over, but the engine hits runaway with those, so one mark over it is...
It still idles kinda rough, even after adjusting the idle speed back and forth looking for the sweet spot.
Third gear has a little more legs up to about 37mph, but beyond that it sounds like the RPMs are up way too high.
I also screwed down the star wheel a tad and rotated the fuel pin to where the deep part faces forward. The truck certainly runs well above 40mph, in fact it absolutely hauls ***. But under 40mph, not so much...
