bad paint questions

So I have one of those early 90's Dodges with the falling off paint. When cleaning that up to repaint, does all the primer have to come off down to bare metal, or was it the paint that was the problem? I think I could get the rest of the bad paint off in about ten minutes with a pressure washer, but the primer seems to be sticking pretty well.

i believe it was the primer, the paint couldnt bond to the primer because it was to hard. i thought that is the problem, i believe ford had the same problem.
