Usually it's just the cord. All that flapping around all year breaks the wires. There are some really cool "Big Rig" plugs that you mount on the bumper. Why not! I think the look like they should be there.

No set temp. It is just on at 750 Watts. You can put it on a timer to come on 2-3 hours before you leave and it will have some bucks on elec. bill. Plug it in when it is below 40F.

Any Cummins shop, big rig store, big truck stop. Check out this thread.

Don't go to the dealer. Any Cummins shop in your area carries parts for your engine much cheaper than the dealers.

Block Heater Links
Tons more if you use the search.

Not always true, it pays to check first. It's been several years but last time I replaced a cord the dealer was cheaper by a whopping $8 @ $24.

Usually the problem is in the cord right behind the AC plug. If you want to save some money just replace the male plug. When mine went out I spliced a higher quality extension cord to the middle of the stock cord and ran it over to the driver's side. Saves a few steps plugging and unplugging which is nice when there's two feet of snow.

You can check the heater itself with an Ohm meter. If it shows any resistance across the connector pins, it's good. A wide open circuit across the pins means it's burnt out.

Test cord

Can also test the cord with a contiuity tester to see if it's got broken wires inside before buying a new one.
