If you are willing to spend the money, I would say get one. I put one on my truck and I love it. I am amazed how much smoother the truck is with it.
I recommend it.

 

defently not a must have but does make a difference in engine feel. i love mine and from what cummins said they help long pushrod engines.

 

FYI ~ A fluidampr type harmonic balancer is standard equipment on certain versions of the 5.9CTD as its maintenance is covered in the Cummins Ops & Maint manual (Industrial B3.9, B4.5, and B5.9 series engines).

I noticed the top rpm's seemed to pull a bit harder with the engine seemingly winding easier at top rpm (3200) when I installed mine.
