Getting hot at idle and low speed

I have a 2003 cummins and was idling and moving around under 5mph with a trailer for about 15 minutes. The coolant guage got to the right edge of the 0's on the 200 mark. So probably got around 215 to 220. Never heard the fan come on until I put in in park and then the rpms went to 1K. Fan will also come on if I run the AC. But I was not using the AC when moving around and the tempo kept rising. As soon as I got above 30mph temp came down pretty quick. Wiring looks fine for the clutch. Could it be my fan clutch even though it works sometimes?
