I would think the problem would most likely exist in the top cover of the transmission, where the shift rails are. You can remove the stick and look at the end of it, and look down its hole for damage. Maybe even try to realign all the rails through the hole, but you will probably end up removing the transmission so you can take the top off.

I Already Looked In There With The Shifter Out And Couldnt See Anyway To Fix It, So I Had It Towed In To The Local Dealer (very Good Freinds Of Mine). Its Just The Damdest Thing I've Seen In All My Years Of Working With These Trucks

It sounds like you lost a roll pin in one of the shift forks. Not a big deal, just goofy as heck.

Not sure how the top of the newer 4500 are put together, but I have dealt with this in several other brands of 5 and 6 speed transmissions.

I've run into this twice on '97s, problem is the bottom of the shifter is worn out allowing you to shift into two gears at once, it gets stuck in fifth.
Very self evident how to fix it with just a screwdriver but it will happen again unless you are very careful shifting.
The big problem is the shifter isn't available separately any more, it's junkyard time.
If the dealer doesn't give you an answer you like ($$$) call the fine folks at Standard Transmission and Gear 800-783-8726
Even the secretary that answers the phone is very familiar with the problem.

In the old days when 5 and 4 trans where in use this happened a lot in the A box. So you had to make your shifts in perfect h patterns or you would be half way up a hill and jammed in the A box with only the main to shift with.
