As far as the fuel foaming, I would try filling a little slower. When does it do it?
Mine foams too. Also all trucks cars have a anti rollover ball in the filler next, doubt the tank is pressurized as I'm almost sure there is a vent nipple on the top of the tank.

Im not sure what you mean about the Roll over value on the top of the truck. It happens everytime I fill the gas tank. I looked at the vent line, no kinks, maybe the plug or stoppage is in the fuel module that lowers into the gas tank. I also noticed that the fuel injection pump has a return line to the gas tank. Maybe the gas cap is not venting and fuel module is plugged inside gas tank..would this cause fuel injection pump to have problems?

Almost the same thing happened to me in my 2000. Would start cold but not once it was warm at all, took a ton of cranking to get it to go.

I found somewhere the coolant temp sensor could give weird readings when going bad and might affect the the computer and starting. It didn't quite make sense to me, but was cheap and I tried it and it worked. Would start much better hot and cold.

when my lift pump went bad it did that. motor died so i called a tow truck. they finally got to about 3 hours later. when they showed just for giggles i tried to start it and it fired right up and ran for several minutes. must have siphoned some from the tank to start but not run. was in a pinch so just replaced it with an original from Geno's. took less than 1 hour from hood up to hood down
