When doing the valve lash, does the guage have to freely slide back and forth? or does it have to be pretty tight?

You want to feel some drag on the feeler gauge, but not enough to make it hard to pull out. Just snug.

How frequently (miles) should we be adjusting our valve lash?

IIRC Cummins list for it to be done every 5 years/150,000 miles.
