Any issues with a 99' 24V?

I am looking at a 99' 24V (would be my 1st 24v) are there any need to know things about this particular year model engine/trans?

Lots of things you need to know, VP44, lift pump, transmissions. The biggest thing is the lift pump and vp44, what is the fuel pressure and when were they replaced. Oh and the vacuum pumps start to leak at 130,000 miles and are fun to fix.

Yep, as stated above.
1.Check the Lift Pump and current fuel psi. If the truck has low fuel psi 0-5 I would question the Injector Pump as they won't last long w/this low of fuel psi.
2. If an auto tranny, check to see if it's been rebuilt and the condition of the fluid...no burned smell, red=good, black=bad. If a rebuild, then you're probaly ok until you start bombing it. No matter what change the fluid once a year or sooner if needed.
3. Check for the 53 series block by looking on the drive side bottom front of the motor. Knock on wood, my 53 block has been good, and there are some out there w/zero issues. Do a search on here for "53 block" and you'll find days worth of reading.
Those are probably the main things to check besides the normal stuff, oil, front-end, driveline, electronics, etc.

Its an auto with 202K...no history on tranny work.

Auto...well I can tell you unless it's been maintained very meticiously, then it's going to need an overhaul soon. With my understanding, the factory auto tranny's have certain parts inside made of plastic! Obviously, after 200k miles theres going to be some wear to these items....especially if it has a gooseneck or 5th wheel hitch OR if it's towed any significant weight via the ball hitch. Just price/buy accordingly as a basic rebuild starts around $3k.

If the block does crack, what are the options for repair? Can you take the guts out of the existing block, re-ring and install new bearings into a replacement block? What would it cost to do the repairs?

You could try the "lock-n-stitch" method but it has not held for some. Yes, I'm guessing you could take the "usable" internal parts from one and build another. Doing the repairs yourself would cost you the time and parts expense, having a quailty cummins rebuild...not sure but I would guess several thousand or more.
