I've got a 92 w350 12v with a stock 727 3spd I want to build it will a 2nd gen flexplate bolt right up?, what should I use for a tourque converter?, what would be the best shift kit?, can I add an o.d. to this one? Someone please help I'm new in the diesel game

Yes the 2nd gen flex plate will work but how in the world did you get a 92 with a 727? Unless some one converted it, it should already have the 518 overdrive in it.

But no, you can't convert a 727 to an O/D. You could add a Gear Vendors O/D to it though.

Would you suggest I start looking for a 518 and build that or is the 727 sufficient

Overdrive is a big plus on these trucks. I hated my 727 for that reason.

Is the 518 o.d. a good trans? Or should I do an Allison conversion

518's are fine with good parts. A 47RH with good parts is a little better as it gives you a lock up converter. Depends on which Alison your talking about.

47RH's are hard to come by and expensive. You can convert a 518 to lockup by following my thread in the sticky.

Take a picture of your trans and post it. Lets make sure you have a 727 before you start spending money you might not need to.

Stock Tranny in a 1992 is a A518, aka 46RH (4 gears, 600FP, Rear, Hydraulic shift) OD is electrical, driven by the PCM which has several inputs it looks at.

To answer your first questions,
1. Yes
2. Goerend.....call them with your application, they'll build exactly what you need.
3. Transgo
4. Mute point

There are several suitable flex plates out there for $150 and up. My son and I both built up a 93, and both got left sitting along side the road with a sheared stock flex plate.

I'm positive it's a727 and I need o.d. and it had water in the trans I had to change my filter and fluid but I dont trust it and if I'm gonna pay to get it rebuilt and its not that good I might as well get the good trans right

Its 4x4 and looks more like the second one but not really its different than both

I bet it looks different as it is actually a A-518 transmission, Dodge put in that year truck.

That's the O/D version, 3 speed with overdrive, and is what Dodge would have put in originally.
