Use the shifter mechanism off an old 10-speed bicycle. The head is just a rotating handle that's held by friction - adjustable by the center-pivot screw. Use one of those ball-chain things to attach the cable to the throttle linkage so it doesn't bind when you're driving ... should cost nothing, just drive around on trash day and grab an old bike for parts.
It's what I use on my ****** to idle up for winching or rock-crawling (those times when you need a third leg to gas, clutch, and brake simultaneously).

That part number worked for me at Napaonline.com, but I did have to take the "-" out of the middle of it. It said there that it was $43 or something, but I think the prices differ from each napa.
