The switch or the resistor. Most likley the switch. A bad resistor will "normally" cause a low speed only situation. Bad switch- no nothin'
Neither one is very expensive. But I believe the resistor is Dealer only.
If you have to go into the dash for either. Jim Lane's Heater on relay trick isn't too bad to do and well worth running the wires while you are in there.

You will still have to fix the heater problem before you do the relay trick. The relays only effect the "HIGH" setting. The 3 lower speeds are still controlled by the factory wiring.
well I got the switch out of parts truck to swap over. Someone had a plastic tie keeping it secured to the heater.
So, lo and behold, I found the reason mine wasn't working was because it had worked it's way loose from the socket.

Quite possible that was there from the factory. I've seen a lot of them that way. glad you found the problem.
