Cab rust and donor truck

I have a 91 D350 and the top of the cab is rusted. I have an option to pick up either a 92 or 93 truck with a good cab. My question is what is the best option fixing the rust. Would it be easier to patch the rust or if either one of the donor trucks would work cut the A-pillars off a good cab a replace the old?

It depend how nice you want it to be in the end.
I did a sort of slap / dash swap on my old '72 after I rolled in on some steep rocky ledges 4 wheeling it year and years ago.
I saw-z-alled off the crushed cab top off my '72 at the pillar posts.
I then cut a '76 with rotted floor boards and cab mounts, top off.
I then spot welded the toupee of the new cab roof on my old '72, installed a roll cage, siliconed a spare windshield in place and went back 4 wheeling the next weekend.
I looked liked carp but I was back wheeling fast and easy and the '72 cab bottom was super solid. So in that case it worked for me. The orange roof welded on the green cab looked funny.
If you are dealing with a daily driver then I would just swap the cabs.

I guess that really answers my question no matter what way I went with it. Either the 92 or 93 cab would work with my 91 if I did a cab swap or cut the roof it would line up.

Basically yes.
The '91 wires differ a little from the '92/'93 wires, so you might have to do some wire swapping as well.
