I have not had one but the US Gear unit appears to be stronger and is rated higher capacity. It also comes either overdrive or underdrive.

My sons 3500 has the Gearvendor under / over drive unit. We used his truck to pull my GN with my truck on it out to Scheids in Indiana weekend before last. That thing is sweet with the 410 rear that his truck has. What hills it didn't want to pull in the high side (mainly due to exhaust temps) it would pull in the low side very well. The rpm difference between the high and low sides in fifth gear is somewhere around 450 to 500. His truck has the dreaded "red" shift button on the gear shift that everybody hates. It works very well. His shift lever also has another red button for the Pac Brake, kinda like driving one of the big rigs in the old days.
