Whats up with this?
About a month ago I let a guy put his smarty on my truck so I could try it out. Since then when I let the clutch out the engine surges, it idles rougher and the cruise control fluctuates from the set speed, goes up and down by about 5mph. So does anyone have any ideas?

Just a guess.
At some point during your trucks life, it had an ECM reflash to fix a surging and/or cruise control problem.
After testing the Smarty, you now have the stock 98.5 ECM program back in there so your surge problem has come back.
Smarty isn't like other programmers that store your current ECM program while running theirs, then put your program back in when done.
It has a whole bunch of stock programs it loads back on when you "return to stock".
This is how they do away with the "remove fuse ##" you have to do with some other programmers.
It's also how they avoid crashing your ECM if it doesn't save your "stock" program just right.

smarty should have loaded the latest and greatest software back into your truck, not sure if thats your problem. Have u talked to bob, he's pry your best bet
