Tranny slipping in first

Tranny slips going in forward in first gear. Reverse is fine. Truck is a 93 w250 automatic. Is the tranny toast? Or can I tighten up anything ? The fluid is full and not burnt

Never heard of that, I'm assuming it has something to do with the t/c draining and not engaging?
