Check all u-joints connected to the driveshaft. I had serious shudder on takeoff as well as a little clunk when putting it in gear. It ended up being the rear u-joint, totally blown. Then, about two months later, the shudder started again, but this time it wouldn't start shuddering til I hit 2nd gear and then it was really bad at about 60-65, started to go away around 70-75. This time it ended up being the middle u-joint, totally blown. I'm waiting for the one by the carrier bearing to go, but not looking forward to changing that one out!

if yours is a long bed quad cab, you've got 3 ujoints and a carrier bearing. the center bearing may not be loose. drive it for a few miles, and then check its temperature with your hand. it'll be hot. It is seizing up.
