P2146 Code Once a Month

OK - I have read various threads on these codes and it could be the wiring or injector. After the third time it happened, I took it to the dealer and they saw the codes in the background, but could not get the truck to trip the codes again (after 2 days). They were nice enough not to charge me for the diagnostic and did not want to throw expensive parts at the truck not knowing exactly what the issue was because they could not replicate it (I was impressed with the dealer).

Here are the symptoms:
March 2011 - I am running the Smarty Jr. on SW2. Driving a couple miles from the house to the bank, I pulled into the bank parking lot and the truck went into limp mode with a hard idle. Plugged in the Smarty JR and got the P2146 code. Started and restarted the truck a couple times and still had the rough idle. Changed the programming back to stock, drove 200 yards and everything went back to normal. Next start up, the check engine light was gone and the codes were gone. Checked all the wiring and it looked OK.

April 2011 - I am back to running Smarty Jr. on SW2. Driving on the freeway to work and the truck starts to run rough for a few seconds, the check engine light comes on, then all of the sudden as fast as it came on, the truck was back to normal. Checked the codes when I got to work and also got the P2146 code again. Started up the truck after work and no check engine light and no code. Checked wires again and no visible issues.

May 2011 - I am still running Smarty Jr. on SW2. Started the truck up after work and as I am driving out of the parking structure, rough idle and limp mode again. This time I got the P2146 and P0203 codes. Tried starting and shutting down the truck a few times. Unplugged the electrical connectors for the injectors at the valve cover (both), blew on them to get any dust out and reconnected. No resolution. Changed the truck back to stock and the codes went away, no check engine light and ran fine.

I am going to keep the truck back on stock for two months to see what happens, but wanted to pose the question to see if anyone has had these intermittent issues and what the end result was and the timing of the end result (e.g. eventual injector failure, electrical harness replacement, ECM replacement, etc.). Thanks in advance for any thoughts.

PMS, maybe?

Seriously, I have no clue. I just recently got into a CR Cummins, and I run it without a programer.
