NV4500 missing washer ?

Something to look for. My brother's 95 1ton 4X4 DCT slipped the 5th gear nut off a week ago and we finely got to fix it this weeken. When we opened up the tail end of the trans. I found the nut and the 5th gear but ???"NO WASHER" ???. I the shop manual it shows a trust washer between the 5th gear and nut. My question is has any one else found this washer missing when they did their repairs to their 5th gear in the NV4500 ? We where the first to open or work on this trans. so the factory must have missed putting the washer in this trans. What had happened to my brother's trans. is the threads on the nut where stripped of, allowing the 5th gear and nut to slide back on the shaft and disingage 5th gear. Only damage was to the nut. We replaced it with the upgraded nut & washer and 8hrs. & $60.00 later where back on the road. AlbertaRAM
