Even without a scanner it is very easy (on '99 up anyway). Take the key and turn it in sequence ON - OFF - ON - OFF - ON. Any stored codes will flash up on the digital odometer. If there is a code 1693 you will need the scanner as there is a "companion code" stored in the ECM.
Things to look for ENGINE wise:
Look for oil leaks, obvious stuff, and for the infamous "53" block.
Click here for more info on the 53 block.
Transmission:
Autos are more common in these truck than are the manuals. The 2 choices for manuals are the NV4500 5 speed and the NV5600 6 speed. The 5 speed have a common problem of losing 5th gear due to the nut that hold the actual gear in place. It gets loose and moves back on the shaft and the gear follows it until it moves far enough to unmesh from the geartrain. And the 5 speed requires special fluid due to its syncros.
The 6 speed is more robust. Prime concerns with it is notchy shifting and hard to get into some gears. More prone in colder areas.
4x4, look up "death wobble" on here. Tons of reading on it. Its mainly an issue with the track bar.

what years are you looking at?? There are some pretty big differences between the second gens (94-2002) and the third gens (03+)
Most people tend to think that the second generation 12Vs are the most reliable engines.
