Sometimes they fail but not very often! We have had 3 trucks with over 250k on them and never had to replace an injector. I bet if you took care of it and ran some additives every now and then, they would last the life of the motor!

I agree with haloman also. I just replaced mine at a little over 100000k and I noticed approx 2mpg gain.
I bought the truck used, so I don't know anything about it prior to me, but I felt the difference from the switch.
I also had a '01 6 speed bought used at 195k, replaced injectors at 220k and got same 2 mpg extra.
I put a lot of miles on my trucks, the slight increase of mileage is very welcome, even if there's the initial cost of injectors.
