I have the Jacob brand on mine, and I like it quite a bit. It is handy for warming up, and great for holding speed down on a grade with a trailer witout dragging brakes.
At idle, it is more of a hiss sound. There is a bit more of a chugging sound when actually used on the road, but it does not sound like a compression brake on a semi...
I have a buddy with a Pac Brake, and he is quite happy with it, too.

Don't think of the "Jake Brake" sound your hear from a semi. It is more of a hiss. Jake Brakes on the big rigs float the valves, an exhaust brake just adds a restriction in the exhaust.
If you tow they are great. I love mine.
