'05 not starting well

Lately, on the first start of the day my truck cranks several times before starting, runs a little rough and smokes a light color for a few seconds. Then it settles down, idles fine and runs great. It only smokes on the first start in the morning and it's been warm enough that the grid heater is not having to come on. Once it's been started the first time all other starts for the rest of the day are fairly normal, with an occasional stumble and cranks once or twice more than it used to but no smoke at all.

A little recent history: This all started after having an oil leak fixed at a shop. they had to replace the gasket behind the gear cover so they had to pull the pump, cam etc. It has been harder to start than normal since I got it back but started better when it was colder and the grid heater was cycling.

Any ideas? May be a fuel pressure problem? Sensor?
There are no codes when I cycle the key 3 times.

That's exactly what my truck did when my injectors were on their way out. They bleed down overnight, flooding the cylinder(s) which causes the smoking when you first start it in the morning. New set of injectors took care of the problem.
