01 nv4500 clunking

Truck has an 01 nv4500 in it that makes a loud clunking in neutral and all gears but when the clutch is pushed in it doesn't make the noise
From what I've heard it has a valair dd clutch and they put the stock needle bearing type back in on the input? Does it need an upgraded intake or could it have blown that bearing out?

Sounds like a bearing or two. You need to pull it apart to be sure.
